For many seniors and their families, the transition from hospital to home can be a stressful time. While most people prefer to return home as soon as possible, hospitals must also ensure that the appropriate level of care and support will be available after discharge. In some situations, a senior may require 24-hour care before discharge can proceed.
SureWork Home Care Solutions can help through our Hospital Discharge Care service, providing the support needed to help make the transition from hospital to home as safe and comfortable as possible.
What We Offer:
24-Hour Care Upon Hospital Discharge in Victoria & Sidney BC
Our staff can meet with families at the hospital or in the home to discuss the level of care required. We help prepare for the return home by arranging 24-hour care following hospital discharge for as long as needed.
This typically involves continuous support provided through two 12-hour shifts per day (day and night), with an emphasis on caregiver consistency whenever possible. If you are looking for more daytime support, with someone available at night to respond in an emergency, the live-in care model may be better suited to your senior care needs.
As care needs change, our services can be adjusted accordingly. If hospital discharge care is no longer required, we can transition to hourly care on an ongoing basis.
